Own the Gate.

Someone or something has to decide if a medicine is appropriate and safe for use. For seventy-five years that was a prescription pad. It is becoming software. An ACNU puts that software on your site, validated by FDA, and keeps the sale there.

01  ·  the handoff

We already pay for the visit. Then we hand it off.

Pharma spends about six billion dollars a year on TV to bring a patient to a brand site. The site cannot sell. It says: talk to your doctor. From there the office, the PBM and the counter own the outcome.

Of every hundred prescriptions written, twenty-eight are never picked up and thirty-six more stop within a year. Thirty-six patients remain.

Hand it off, and someone else owns the gate and the counter.

Two jobs are irreducible.
Both are moving.

Strip drug access to the studs and two jobs remain: someone must pay, and something must decide it is safe. Everything else is scaffolding. Both are changing hands at once.

Routine medicine will be consumer-funded and software-gated.Both shifts are already underway. The question is whose software ends up at the gate, and who validated it.

03  ·  the gate with a pathway

One gate. Every front door.

An Additional Condition for Nonprescription Use, an ACNU, is an FDA-approved questionnaire a consumer answers to buy a medicine without a prescription. It lives in the NDA, so it can’t drift. The front door is moving: 300 million people a week ask ChatGPT about their health, but the purchase still happens on the brand’s site. The agent layer carries the same questionnaire to every front door and sends qualified consumers to your counter.

04  ·  door · gate · counter

We build all three.

An ACNU program is a feasibility question, a validated piece of software and a storefront. The leaderboard answers the first for every brand. TACTiC proved the second in an actual-use study.

The pathway is young. The work is not hypothetical.

06  ·  the clock

Leave the counter before the cliff.

On day one of generic entry the counter swaps the brand out. A year later it holds about a fifth of its units. A nonprescription launch at LOE minus one takes the shelf first and turns refills into subscriptions the counter cannot substitute. Feasibility to launch is about four years. Three clocks are running. The sponsor sets none of them.
